Berberine

Also searched as: berberine HCl · berberine hydrochloride · goldenseal alkaloid · barberry extract · Coptis

A plant compound that activates AMPK in lab studies. A 2020 meta-analysis found modest weight loss, but its authors say the evidence cannot firmly support it.

Spotlight

Weight
About 2 kg (4.6 lb) average loss across 12 randomized trials (1,040 people), with a confidence interval of 1.05 to 3.09 kg.
Mechanism
Reported to activate AMPK, a cell energy sensor, in lab studies.
Longevity
Lab and animal evidence only. We found no human lifespan trial.

Risk to know

Overactive AMPK

AMPK can switch off growth signals used for muscle building. Berberine has not been tested for this in people, so this is a caution, not a finding.

  • AMPK can suppress mTOR, the growth signal behind muscle protein building. Whether berberine blunts gains from strength training has not been tested in people.
  • Combined with metformin, insulin or other glucose-lowering drugs, berberine could push blood sugar too low. Check with a pharmacist or clinician.
  • People who are underweight, frail, or trying to build muscle in older age should talk to a clinician before using it long term.

Telomere details

What is known about Berberine and telomeres

No human telomere data found

How it is meant to work

AMPK activation has been proposed to slow telomere wear. Berberine does not act on telomerase directly.

What has been measured

  • We found no human trial that measured telomere length after berberine.

What it means

Any telomere benefit is a hypothesis. Its human evidence is for weight and related markers, not telomeres.

Telomeres are the protective caps on chromosomes. Blood-cell telomere length is a biomarker; it has not been shown to predict how skin looks or how long you live.

What is Berberine?

Berberine is a yellow alkaloid found in plants such as goldenseal, barberry and Coptis. Supplements usually contain berberine hydrochloride.

Why it comes up for skin and aging

Berberine is studied for metabolism rather than skin. We found no skin trial.

What the research shows

Asbaghi et al. 2020 (Clinical Nutrition ESPEN) pooled 12 randomized trials with 1,040 volunteers (518 berberine, 522 control). Berberine reduced body weight by 2.07 kg (95% CI −3.09 to −1.05), BMI by 0.47 kg/m², waist circumference by 1.08 cm, and CRP by 0.42 mg/L. It had no significant effect on ALT or AST.

The authors state that their analysis "cannot support the beneficial effects of berberine supplementation" on body weight, BMI, waist circumference and CRP, citing the small number of studies and heterogeneity between them.

In lab studies, berberine is reported to activate AMPK, a cell energy sensor that fasting and exercise also activate. That is a mechanism, not evidence of a fasting effect in people.

We found no human trial of berberine and lifespan. Longevity claims rest on lab and animal work.

Forms, amounts and limits

Doses and products vary widely. Follow the product label and ask a pharmacist before combining it with medicines.

Safety

NCCIH says that when goldenseal is taken by mouth, very little berberine may be absorbed. It also says the berberine constituent of goldenseal can be harmful to newborns, and advises pregnant or breastfeeding women against using goldenseal.

Berberine can interact with drugs cleared by liver enzymes. Check with a pharmacist if you take any prescription medicine.

Talk to a doctor or pharmacist before starting any supplement, especially if you are pregnant, breastfeeding, have a medical condition or take medicines.

Questions

Does berberine mimic fasting?

In lab studies, berberine is reported to activate AMPK, the energy sensor that fasting and exercise also activate. That does not show it has the effects of a fast in people.

How much weight can berberine help you lose?

A 2020 meta-analysis of 12 randomized trials found about 2 kg (4.6 lb) on average. The authors say the evidence cannot firmly support that benefit.

Does berberine extend lifespan?

We found no human lifespan trial. The longevity evidence is from lab and animal studies.

What is the risk of too much AMPK activation?

AMPK can switch off growth signals used for muscle building. Berberine has not been tested for this in people. Combined with diabetes medicines, it could push blood sugar too low. Talk to a clinician before long-term use.
